Balsam, particularly balsam of Peru and balsam of Tolu, has a long history of traditional use in treating skin conditions, including abscesses. In various folk medicine systems, balsams have been applied topically to wounds, ulcers, and abscesses due to their perceived antiseptic and healing properties. Historical texts and ethnobotanical records indicate that balsam was valued for its ability to promote wound healing and reduce infection, likely because of its aromatic resins and essential oils, which have mild antimicrobial effects when tested in vitro. However, there is limited modern clinical evidence supporting its efficacy for abscess treatment; most support comes from traditional usage rather than rigorous scientific studies. Some in vitro studies have demonstrated antibacterial activity of balsam extracts against certain bacteria, but these findings have not translated into standardized medical practices, and concerns about allergic reactions and contact dermatitis have limited its use in contemporary medicine. Therefore, while balsam’s use in abscess management is rooted in tradition and supported by some laboratory evidence, it lacks robust clinical validation, and its application is not widely recommended in modern medical guidelines.
