Hydrogen chloride (HCl), when dissolved in water, forms hydrochloric acid, which is the primary acidic component of gastric juice in the human stomach. The secretion of HCl by the parietal cells of the stomach is essential for digestion. It lowers the gastric pH, denatures proteins, activates the enzyme pepsinogen to pepsin, and provides defense against ingested pathogens. Hypochlorhydria (low stomach acid) can impair digestion and nutrient absorption. Supplemental betaine hydrochloride and other forms of HCl have been used clinically to support individuals with low stomach acid, although the primary evidence base relates to the physiological necessity of HCl for normal stomach function rather than supplementation per se.

Scientific studies confirm the crucial role of endogenous hydrochloric acid in digestion and absorption of various nutrients, particularly vitamin B12, iron, and calcium. However, evidence for the efficacy and safety of supplemental HCl (such as betaine HCl) is less robust, with only a few clinical trials and case reports documenting increased gastric acidity and improved digestive symptoms in select individuals with diagnosed hypochlorhydria or after gastric surgery. The use of HCl supplements is not recommended for those with normal acid production or gastrointestinal ulcers due to potential risks. In summary, the role of HCl in supporting the stomach is scientifically established, but the evidence for supplementation is more limited and should be approached with caution.
