Thiosulfinates are sulfur-containing compounds most notably found in garlic (Allium sativum) and onions. The primary thiosulfinate in garlic is allicin, which is formed enzymatically when garlic is crushed. Allicin and related thiosulfinates are known for their broad-spectrum antimicrobial activity, including effects against oral pathogens implicated in gingivitis, such as Porphyromonas gingivalis and Streptococcus mutans. Several in vitro studies have demonstrated that thiosulfinates can inhibit the growth of these bacteria and reduce inflammation-related pathways. Limited clinical studies have evaluated topical garlic extracts or thiosulfinate-rich mouthwashes and reported reductions in gingival inflammation and plaque indices, though the studies are generally small, short-term, and methodologically weak.

Despite promising laboratory data, the clinical evidence for thiosulfinates in gingivitis treatment is still emerging and far from robust. There are no large-scale, high-quality randomized controlled trials that establish efficacy comparable to standard treatments such as chlorhexidine or mechanical plaque removal. However, mechanistic plausibility (antimicrobial and anti-inflammatory actions) and some positive pilot results support ongoing research interest. The use of thiosulfinates for gingivitis is thus primarily based on preliminary scientific findings rather than tradition or widespread clinical validation.
