Calamus (Acorus calamus), also known as sweet flag, has a long history of use in traditional medicine systems such as Ayurveda and Traditional Chinese Medicine (TCM). In these traditions, calamus rhizome is used for various respiratory ailments, including congestion and sinus issues. The herb is thought to act as an expectorant and to help clear mucus from the respiratory tract. However, scientific studies validating these specific uses are lacking. There is minimal modern pharmacological research directly investigating calamus for sinus congestion or related conditions. Most available evidence is anecdotal or based on traditional herbal practices, where preparations of calamus may be inhaled, made into teas, or used as powders. In addition, safety concerns exist: calamus contains β-asarone, a compound that has shown carcinogenicity in animal studies, which limits its use in some countries. Thus, while calamus has a traditional role in treating congestion, there is little in the way of robust clinical or scientific evidence to support its efficacy for this specific purpose.
