Belleric myrobalan (Terminalia bellirica), known as "Bibhitaki" in Ayurveda, has a long history of traditional use in Indian and other South Asian medical systems. It is one of the three fruits in the classic Ayurvedic formula Triphala. In traditional practice, Belleric myrobalan is used to address respiratory conditions, including cough, sore throat, and congestion of the lungs. The fruit is believed to have expectorant, anti-inflammatory, and antimicrobial properties according to Ayurvedic texts such as the Charaka Samhita and Sushruta Samhita. These texts describe its ability to help expel phlegm and clear respiratory passages, which underpins its use for lung congestion and related symptoms.

However, scientific validation of these effects is limited. While some in vitro and animal studies suggest possible antimicrobial and anti-inflammatory activities of Belleric myrobalan extracts, direct evidence for its efficacy in treating lung congestion or supporting respiratory health in humans is lacking. Few clinical trials exist, and those available are of low methodological quality or combine Belleric myrobalan with other herbs, making it difficult to attribute effects solely to this ingredient. Therefore, its use for supporting or treating lung congestion is primarily justified by traditional knowledge, with modest preclinical support but insufficient clinical evidence.
