Methylxanthines, such as theophylline and aminophylline, have a well-established scientific basis for use in respiratory conditions characterized by bronchial congestion and obstruction, such as asthma and chronic obstructive pulmonary disease (COPD). These compounds act primarily as nonselective phosphodiesterase inhibitors, leading to increased intracellular cyclic AMP levels, which in turn cause bronchodilation and relaxation of airway smooth muscle. Additionally, methylxanthines possess mild anti-inflammatory effects and may improve mucociliary clearance, contributing to the alleviation of bronchial congestion. Historically, theophylline was one of the mainstay treatments for asthma and other obstructive airway diseases before the widespread adoption of inhaled beta-agonists and corticosteroids. However, its use has declined due to a narrow therapeutic window and the risk of side effects. Clinical guidelines currently reserve methylxanthines for specific situations, such as refractory cases or as adjunctive therapy. There is moderate-quality evidence from randomized controlled trials supporting their efficacy in improving lung function and symptoms in patients with bronchial congestion due to obstructive pulmonary diseases, though the effect size is generally modest compared to newer therapies.
