English Ivy (Hedera helix) leaf extract has a long history of use in traditional herbal medicine for relieving bronchial congestion and coughs. More recently, a number of clinical studies and systematic reviews have evaluated its efficacy in this context. The active compounds, primarily saponins such as hederacoside C, are thought to help loosen mucus in the airways and facilitate expectoration, potentially aiding in the relief of cough and congestion associated with respiratory tract infections.

Several randomized controlled trials (RCTs) and observational studies have examined standardized English Ivy extracts, often in pediatric and adult patients with acute bronchitis or productive cough. These studies, while generally of moderate methodological quality, have reported improvements in symptoms such as cough frequency, sputum production, and airway inflammation compared to placebo or standard care. A 2017 systematic review published in Phytomedicine found that English Ivy extracts were associated with a statistically significant reduction in cough frequency and improvement in respiratory symptoms, although the authors noted that many studies had limitations such as small sample sizes and short durations.

Overall, while the evidence is not uniformly strong and high-quality, there is moderate scientific support for the use of English Ivy in alleviating bronchial congestion, particularly as an adjunctive therapy. Its use is recognized in some European phytotherapy guidelines for acute bronchitis and cough.
