Andrographolide, the main bioactive compound in Andrographis paniculata, has been traditionally used in Asian medicine for respiratory complaints, including bronchial congestion. In recent years, several clinical trials and reviews have investigated its efficacy for upper respiratory tract infections (URTIs), which can involve symptoms of bronchial congestion. Meta-analyses and randomized controlled trials have found that standardized extracts of Andrographis may reduce symptom severity and duration in acute respiratory infections, including cough and sore throat. Proposed mechanisms include anti-inflammatory, antiviral, and immunomodulatory effects, which could theoretically support relief of bronchial congestion by reducing airway inflammation and mucus production.
However, most studies focus on general respiratory symptoms or uncomplicated URTIs rather than specifically targeting bronchial congestion or bronchitis. Evidence directly assessing its effect on bronchial congestion is limited and of moderate quality. The World Health Organization and some national herbal pharmacopeias recognize Andrographis for symptomatic relief of uncomplicated respiratory infections, but not specifically for bronchial congestion. Overall, while the traditional use is supported by some modern clinical data, the direct scientific evidence for bronchial congestion is modest, justifying a rating of 2/5.
