Neoandrographolide is a diterpenoid compound found in Andrographis paniculata, a plant traditionally used in Asian medicine for treating respiratory tract infections, including the common cold. While andrographolide is the primary active compound, neoandrographolide is structurally related and present in significant amounts, contributing to the overall pharmacological activity of the extract.

Several in vitro studies indicate that neoandrographolide possesses antiviral properties, including inhibition of viral replication in certain respiratory viruses. Animal studies and some cell-based assays have demonstrated that extracts containing neoandrographolide can reduce viral titers and inflammation associated with respiratory infections. However, most clinical trials on colds utilize the whole Andrographis paniculata extract rather than isolated neoandrographolide. These studies suggest modest effectiveness in reducing the severity and duration of cold symptoms, but they do not distinguish the effects of neoandrographolide from other constituents.

Overall, while there is some scientific evidence supporting the antiviral activity of neoandrographolide and its parent plant against cold viruses, direct clinical evidence for isolated neoandrographolide is limited. The use of neoandrographolide for colds is therefore partially supported by scientific evidence, but the quality and amount of specific data on the isolated compound is low to moderate.
