Vitamin B6 (pyridoxine) has scientific support for its use in reducing certain side effects of chemotherapy, particularly chemotherapy-induced peripheral neuropathy (CIPN) and hand-foot syndrome (palmar-plantar erythrodysesthesia), which are common complications from agents such as cisplatin, oxaliplatin, and capecitabine. Several clinical studies and randomized controlled trials have investigated the efficacy of vitamin B6 supplementation (typical doses range from 50-200 mg/day) in preventing or lessening the severity of neuropathy and skin toxicity in cancer patients undergoing chemotherapy.

A 2012 randomized controlled trial published in the Journal of Clinical Oncology reported that vitamin B6 supplementation reduced the incidence and severity of hand-foot syndrome in patients receiving capecitabine. Other studies, including a 2020 meta-analysis in Supportive Care in Cancer, suggest that vitamin B6 may have a modest benefit in reducing the severity of CIPN, although results are mixed and not all trials have shown statistically significant effects. The proposed mechanism for vitamin B6's benefit involves its role in nerve function and repair, as well as its anti-inflammatory properties.

While vitamin B6 is not universally recommended as standard care for all chemotherapy patients, its generally safe profile and some positive clinical evidence have led to its use in supportive oncology settings. However, high doses of vitamin B6 can themselves cause neuropathy, so dosing and duration should be closely monitored.
