Testolone, also known as RAD140, is a selective androgen receptor modulator (SARM) that has gained attention in the fitness and bodybuilding communities for its potential to promote lean muscle mass and strength. The scientific rationale for its use in supporting weight gain—specifically, lean body mass—stems from its mechanism of action. Testolone selectively binds to androgen receptors in muscle and bone tissues, mimicking some of the anabolic effects of testosterone without many of the undesirable side effects linked to traditional anabolic steroids.

Preclinical animal studies and limited early human trials have demonstrated that RAD140 can increase muscle mass and decrease fat mass. For instance, a Phase I clinical trial (2017) investigated RAD140 in postmenopausal women with breast cancer and reported increases in lean muscle mass as a secondary finding. However, comprehensive clinical trials specifically evaluating RAD140 for weight gain or muscle wasting conditions are still lacking.

Currently, RAD140 is not approved by the FDA for any medical use, and its safety profile is not fully characterized in humans. Its use is largely experimental and often occurs outside of medical supervision. While there is emerging scientific evidence supporting its anabolic potential, the overall strength of evidence is moderate (rated 3/5), and more robust, long-term studies are needed to fully validate its efficacy and safety for supporting weight gain in clinical populations.
