Testolone (RAD140) is a selective androgen receptor modulator (SARM) that has been investigated for its potential to treat muscle wasting (cachexia) associated with conditions such as cancer, chronic illness, and aging. The scientific rationale for its use is based on its ability to selectively stimulate androgen receptors in muscle and bone tissues, promoting anabolic activity (muscle growth) while minimizing the androgenic effects (such as prostate enlargement) seen with traditional anabolic steroids.

Several preclinical studies in animal models have demonstrated that RAD140 can increase lean muscle mass and improve muscle strength without significant adverse effects on prostate or other androgen-sensitive tissues. Early phase clinical trials in humans have shown that Testolone is generally well tolerated and may increase lean body mass in healthy volunteers or patients with muscle wasting, although the sample sizes have been small and the data is still preliminary.

As of 2024, Testolone is not approved for medical use by any major regulatory authority, and more robust, large-scale clinical trials are needed to establish its efficacy and safety profile in the context of muscle wasting disorders. Nevertheless, the existing scientific evidence provides a rationale for its investigation in this area, and ongoing studies may further clarify its therapeutic role.
