MK-677 (also known as ibutamoren) is a non-peptide agonist of the ghrelin receptor that stimulates the secretion of growth hormone (GH) and insulin-like growth factor 1 (IGF-1). There is scientific evidence supporting its use for the treatment of wasting conditions, particularly muscle wasting (cachexia) associated with chronic illnesses such as cancer, chronic obstructive pulmonary disease (COPD), and aging (sarcopenia). Several phase II clinical trials have demonstrated that MK-677 increases serum GH and IGF-1 levels, which can lead to an increase in lean body mass. For example, a pivotal study published in JAMA (2008) showed that elderly adults treated with MK-677 for 12 months had increased fat-free mass and improved GH/IGF-1 profiles, although functional outcomes were less clear. Other studies in patients with wasting or catabolic conditions have also reported increases in appetite and weight. However, data on improvements in muscle strength or functional outcomes remain limited, and long-term safety is still under investigation. The evidence is promising but not definitive, which is why the evidence rating is 3/5; MK-677 is not yet approved by regulatory agencies for this indication, but it is the subject of ongoing research.
