Ibutamoren (also known as MK-677) is a non-peptide agonist of the ghrelin receptor that stimulates the endogenous secretion of growth hormone (GH) and insulin-like growth factor 1 (IGF-1). There is scientific interest in its use for treating or supporting conditions characterized by wasting, such as muscle wasting, cachexia, and age-related sarcopenia. Clinical studies have demonstrated that Ibutamoren increases circulating GH and IGF-1 levels, leading to an increase in lean body mass. For example, a double-blind, placebo-controlled study in healthy elderly subjects showed that daily oral administration of Ibutamoren for 12 months significantly increased lean mass and improved markers of muscle strength and function (Murphy et al., J Clin Endocrinol Metab, 2001). Similar results have been observed in patient populations with wasting due to chronic illness or bed rest. However, while increases in muscle mass are consistently reported, results regarding improvements in strength and functional outcomes are mixed, and long-term safety data are limited. Ibutamoren is not currently approved by regulatory agencies for the treatment of wasting disorders, but ongoing research supports its potential utility in this area. Thus, the evidence is scientific, with a moderate level of support (rating: 3) based on human clinical trials, but more research is needed to establish efficacy and safety for routine clinical use.
