Creatine alpha-ketoglutarate is a compound combining creatine with alpha-ketoglutarate (AKG). While creatine monohydrate has substantial evidence supporting its use in increasing muscle mass and strength, especially in the context of muscle wasting (sarcopenia, cachexia, etc.), there is less direct evidence for the specific combination of creatine with alpha-ketoglutarate. However, the rationale for its use in treating or supporting wasting conditions comes primarily from the well-established role of creatine in muscle energy metabolism and the theoretical benefit of AKG in supporting the Krebs cycle and amino acid metabolism.

Several clinical trials have shown that creatine supplementation can help increase lean body mass and muscle function in patients with muscle-wasting conditions, such as muscular dystrophies, aging-related sarcopenia, and some chronic diseases. AKG, meanwhile, is thought to possibly enhance creatine absorption or provide its own metabolic benefits, but direct evidence for the creatine-AKG combination is limited. Most studies use creatine monohydrate, not the alpha-ketoglutarate form.

Therefore, while the use of creatine (in general) for wasting has some scientific support, there is little direct clinical evidence for creatine alpha-ketoglutarate specifically.
