Lactobacillus species are commonly used as probiotics to support or treat vaginitis, particularly bacterial vaginosis (BV) and vulvovaginal candidiasis (VVC). The rationale for their use is grounded in the scientific understanding that a healthy vaginal microbiota is typically dominated by Lactobacilli, which help maintain an acidic environment (pH <4.5) and inhibit the growth of pathogenic organisms through the production of lactic acid, hydrogen peroxide, and bacteriocins.

Several randomized controlled trials and meta-analyses have investigated the efficacy of oral and intravaginal Lactobacillus supplementation in the prevention and treatment of vaginitis. The results are mixed but generally suggest a modest benefit, particularly as an adjunct to standard antibiotic or antifungal therapy. For example, a 2021 Cochrane review concluded that probiotics containing Lactobacilli may improve cure rates for BV when used alongside antibiotics, though the quality and consistency of evidence vary. Some studies also report a reduction in recurrence rates for both BV and VVC.

However, the evidence is not uniformly strong (hence a rating of 3). Variability in probiotic strains, dosages, delivery routes, and study methodologies makes it difficult to draw firm conclusions. While the scientific rationale is sound and there is some clinical data to support their use, more high-quality, large-scale studies are needed to establish the efficacy and optimal use of Lactobacillus probiotics for vaginitis.
