Estrogen is scientifically validated as a treatment for vaginal dryness, particularly in postmenopausal women. Vaginal dryness often arises due to a decrease in estrogen levels, which leads to thinning and reduced elasticity of the vaginal epithelium, as well as diminished vaginal secretions. Numerous randomized controlled trials and meta-analyses have shown that both systemic (oral, transdermal) and local (vaginal creams, tablets, rings) estrogen therapies significantly improve symptoms of vaginal dryness, dyspareunia (painful intercourse), and vaginal atrophy. Local estrogen therapy is commonly recommended as the first-line treatment because it delivers the hormone directly to the affected tissues with minimal systemic absorption, thus reducing the risk of systemic side effects.

Authoritative guidelines, including those from the North American Menopause Society (NAMS) and the American College of Obstetricians and Gynecologists (ACOG), endorse estrogen therapy for moderate to severe symptoms of vaginal atrophy and dryness when non-hormonal treatments are insufficient. These recommendations are based on extensive clinical evidence demonstrating improvement in both objective measures (such as vaginal pH and epithelial maturation) and subjective symptoms. Estrogen therapy is one of the most studied and effective interventions for this condition and is supported by decades of clinical use and research.
