Peony, particularly Paeonia lactiflora, has a long history of use in traditional Chinese medicine (TCM) for supporting female reproductive health, including conditions associated with the uterus such as menstrual irregularities, dysmenorrhea, and infertility. In TCM, peony root (commonly called "Bai Shao") is believed to nourish the blood, regulate menstruation, and alleviate pain. It is frequently combined with other herbs (notably in formulas like Shao Yao Gan Cao Tang and Dang Gui Shao Yao San) to address gynecological complaints.

Scientific studies offer some support for these traditional uses, though the evidence is not robust. Preclinical research has indicated that peony root extracts contain bioactive compounds (such as paeoniflorin) with anti-inflammatory, antispasmodic, and hormone-modulating properties. Some small clinical trials, typically in combination with other herbs, suggest benefits in reducing menstrual pain and improving symptoms of polycystic ovary syndrome (PCOS). However, there is a lack of large, well-controlled human studies isolating peony as the sole active ingredient for uterine or reproductive health. Therefore, while the traditional use is strong and there is some preliminary scientific backing, more research is needed for firm conclusions.
