Diosgenin is a naturally occurring steroidal sapogenin found in various plants, most notably wild yam (Dioscorea species). Traditionally, diosgenin-containing plants have been used in folk medicine to support women's reproductive health, including uterine function, menstrual regularity, and menopausal symptoms. These uses are particularly common in traditional Chinese medicine and Native American herbalism. The rationale behind its traditional use is based on the fact that diosgenin is a precursor for the laboratory synthesis of steroid hormones such as progesterone and estrogen. However, in the human body, diosgenin itself is not converted into these hormones naturally; this transformation requires industrial chemical processes.

Scientific research on diosgenin's direct effect on the uterus is limited. Some in vitro and animal studies suggest that diosgenin may exhibit weak estrogenic or anti-inflammatory activity, which could theoretically influence uterine health. However, these effects have not been conclusively demonstrated in human clinical trials. The bulk of evidence supporting diosgenin for uterine health remains anecdotal or based on traditional practices rather than robust scientific validation. Therefore, while there is a traditional basis for using diosgenin to support the uterus, modern scientific evidence is insufficient to strongly endorse its efficacy for this purpose.
