The rationale is based on the ability of certain Lactobacillus strains to colonize the vaginal and periurethral mucosa, thereby restoring or maintaining a healthy microbiota that can inhibit uropathogen adhesion and growth. Clinical trials and systematic reviews suggest that orally or vaginally administered Lactobacillus (such as L. rhamnosus, L. reuteri, and L. crispatus) may reduce the recurrence of UTIs, though the effect sizes are generally modest and results are somewhat inconsistent across studies. Meta-analyses (e.g., Grin et al., 2013; Schwenger et al., 2015) recognize the biological plausibility and some benefit, but emphasize the need for larger, high-quality randomized controlled trials to clarify efficacy, optimal strains, and delivery methods. Currently, guidelines do not universally endorse probiotics for UTI prevention due to these limitations, but they are considered a low-risk adjunct or alternative to antibiotics, especially for those seeking non-antibiotic options.
