Lactobacillus gasseri is a probiotic bacterium that is naturally found in the human genitourinary and gastrointestinal tracts. Its use in supporting or treating urinary tract infections (UTIs) is primarily based on scientific investigations, although the strength of evidence remains moderate and somewhat preliminary (evidence rating: 2/5). Several studies have identified that women with recurrent UTIs tend to have a reduced population of Lactobacillus species, including L. gasseri, in their vaginal flora. The proposed mechanism is that L. gasseri may help restore and maintain a healthy microbiota, outcompete uropathogenic bacteria (like Escherichia coli), and produce substances such as lactic acid and bacteriocins that inhibit pathogen colonization.

Clinical research is limited but promising. A few small-scale clinical trials and in vitro studies have shown that L. gasseri can inhibit the growth of uropathogens and may reduce the incidence of recurrent UTIs when administered as part of probiotic formulations, especially when delivered vaginally. However, most research has focused on the broader group of lactobacilli rather than L. gasseri specifically, and results are not always consistent. Large, well-controlled clinical trials are still lacking.
