Hyperimmune milk peptides (HMP), derived from cows immunized against specific pathogens, have been investigated for their potential to support urinary tract health, including prevention or adjunctive treatment of urinary tract infections (UTIs). The scientific rationale is based on the presence of immunoglobulins and other bioactive components in the milk, which may help neutralize uropathogens or modulate mucosal immunity. Several small clinical trials and pilot studies have assessed the use of hyperimmune bovine colostrum or milk products, particularly those targeting Escherichia coli, the most common cause of UTIs. Some studies suggest that these peptides can reduce bacterial adhesion to uroepithelial cells or may decrease UTI recurrence rates, especially in women prone to recurrent infections. However, the body of evidence remains limited: most studies have small sample sizes, short follow-up durations, and in some cases, lack placebo controls or blinding. No large, definitive randomized controlled trials have established HMP as a standard or primary therapy for UTIs. Regulatory authorities do not recognize hyperimmune milk peptides as an approved treatment for UTIs, and most clinical guidelines do not mention their use. Thus, while there is a scientific basis and some preliminary clinical data suggesting potential benefit, the overall quality and quantity of evidence is low, meriting a rating of 2 on the 0-5 scale.
