Potassium citrate malate is primarily justified by scientific evidence for supporting the urinary system, particularly in the management of kidney stone disease. Potassium citrate, a closely related compound, is well-established in clinical practice for the prevention of calcium oxalate and uric acid kidney stones. The mechanism involves increasing urinary citrate excretion, which binds to urinary calcium, thus reducing the formation of insoluble calcium salts. This effect also helps alkalinize the urine, which can further reduce the risk of certain types of stones.

Potassium citrate malate, while less extensively studied than potassium citrate alone, is believed to provide the same physiological benefits since citrate and malate are both metabolized to bicarbonate, aiding in urine alkalinization. Several clinical trials and reviews (e.g., Pak, 1986; Curhan, 2001; Worcester & Coe, 2010) support the use of potassium citrate for reducing stone recurrence. Its use is recommended in guidelines for patients with hypocitraturia or recurrent nephrolithiasis.
