Phyllanthus fraternus, a herb closely related to Phyllanthus niruri, has been traditionally used in Ayurvedic and folk medicine to address urinary system disorders, particularly for its purported benefits in supporting kidney and urinary tract health. The scientific evidence supporting its use primarily comes from studies on P. niruri and related species, with P. fraternus sometimes specifically cited. Research has demonstrated potential diuretic, anti-urolithiatic (anti-kidney stone), and hepatoprotective effects. Several animal studies have shown that extracts of P. fraternus can reduce the formation of kidney stones through inhibition of stone nucleation and promotion of stone expulsion. Additionally, some in vitro and in vivo studies indicate antioxidant and anti-inflammatory properties, which may indirectly benefit the urinary system by reducing oxidative stress and inflammation associated with urinary tract disorders.

However, while the preclinical evidence is promising, there is a lack of large-scale, well-controlled human clinical trials specifically on P. fraternus. Most clinical evidence comes from studies on P. niruri, which share many phytochemical constituents. Therefore, while the use of P. fraternus for urinary system support is scientifically plausible and is supported by preliminary research, the overall strength of direct clinical evidence is moderate, warranting a rating of 3. Further research, particularly human trials, is needed to fully validate its efficacy and safety profile for urinary health.
