Buchu (Agathosma betulina) has a long history of traditional use in South African herbal medicine, especially among indigenous Khoisan people, as a remedy for various urinary tract ailments. Traditionally, the leaves were brewed as a tea or infused in brandy and taken to alleviate symptoms of urinary tract infections (UTIs), cystitis, and kidney issues. Early European settlers in South Africa adopted its use, and by the 19th century, buchu became a popular botanical in Western herbalism for urinary complaints.

In terms of scientific validation, the evidence is limited. Buchu contains volatile oils such as diosphenol, which have been suggested to possess antiseptic and diuretic properties. Some in vitro studies have demonstrated mild antimicrobial activity of buchu extracts against certain urinary pathogens, but the clinical relevance of these findings is unclear. Human trials are lacking, and there is insufficient rigorous research to support its efficacy for urinary tract health beyond its diuretic effect. Thus, while its use is entrenched in traditional medicine and has some plausible mechanisms, the overall evidence base remains weak, and it is best characterized as a traditional remedy with limited scientific support.
