The use of Malabar Nut Tree Leaf Extract, derived from Justicia adhatoda (also known as Vasaka or Adhatoda vasica), to support or treat tuberculosis is primarily grounded in traditional medicine systems such as Ayurveda and Unani. In these traditions, the leaves have been employed for centuries to manage respiratory ailments, including cough, bronchitis, and symptoms associated with tuberculosis. The active alkaloid, vasicine, has been recognized for its bronchodilator, expectorant, and antimicrobial properties, which are thought to help ease respiratory distress and facilitate mucus expulsion.

However, while some in vitro and animal studies have indicated possible antimycobacterial and immunomodulatory effects of its constituents, robust clinical evidence directly supporting its efficacy against Mycobacterium tuberculosis in humans is limited.
