Guggulsterones are bioactive compounds derived from the resin of the Commiphora mukul tree, traditionally used in Ayurvedic medicine for various lipid disorders. There is some scientific evidence suggesting that guggulsterones may help lower triglyceride levels. Early clinical studies from India in the 1980s and 1990s suggested that guggul extracts could reduce serum triglyceride and cholesterol levels. The proposed mechanism involves modulation of nuclear hormone receptors such as the farnesoid X receptor (FXR), which is implicated in cholesterol and bile acid metabolism.

However, more recent and rigorous clinical trials, primarily outside of India, have reported mixed or negative results. For example, a well-designed randomized controlled trial published in JAMA (2003) found that guggulsterones did not significantly reduce serum cholesterol or triglycerides, and in some cases, lipid profiles worsened. Reviews and meta-analyses generally conclude that while there is some mechanistic rationale and limited positive evidence, the clinical efficacy of guggulsterones for hypertriglyceridemia is unproven and inconsistent.

In summary, while traditional use and some early studies suggest a possible triglyceride-lowering effect, the current body of scientific evidence is weak and does not strongly support guggulsterones as an effective treatment for high triglycerides. More high-quality trials are needed to clarify their role, if any, in lipid management.
