Alpha lipoic acid (ALA) is a naturally occurring antioxidant that has been investigated for its potential role in improving lipid profiles, including lowering elevated triglyceride levels. The scientific evidence supporting its use for hypertriglyceridemia is modest but emerging. Several small-scale clinical trials and animal studies have suggested that ALA supplementation can lead to reductions in triglyceride levels, particularly in individuals with metabolic syndrome, type 2 diabetes, or non-alcoholic fatty liver disease. The proposed mechanisms include improved insulin sensitivity, enhanced fatty acid oxidation, and decreased hepatic lipogenesis (fat production in the liver). A meta-analysis published in 2019 found a statistically significant, though modest, reduction in triglyceride concentrations with ALA supplementation compared to placebo.
However, the quality and size of clinical trials are variable, and some studies have not shown significant effects. The overall body of evidence is still limited, and larger, high-quality randomized controlled trials are needed to confirm these findings and establish optimal dosing and long-term safety. Despite these limitations, ALA is sometimes recommended as an adjunctive therapy for individuals with elevated triglycerides, especially when associated with insulin resistance or metabolic disorders. At present, ALA should not be considered a first-line treatment for high triglycerides, but it may offer some benefit as part of a comprehensive lifestyle and medical management plan.
