Andrographis paniculata, commonly known as "king of bitters," has a long history of use in traditional medicine systems such as Ayurveda and Traditional Chinese Medicine for treating upper respiratory tract infections, including conditions like tonsillitis. In recent decades, several randomized controlled trials and meta-analyses have evaluated Andrographis for acute respiratory tract infections, including pharyngotonsillitis. The active compound, andrographolide, is thought to have anti-inflammatory, immunomodulatory, and antiviral properties. Studies have demonstrated that Andrographis extracts may reduce the severity and duration of sore throat and related symptoms compared to placebo, with some trials specifically including patients with tonsillitis or pharyngotonsillitis. For example, a systematic review published in the journal Phytomedicine (2017) concluded that Andrographis is more effective than placebo for symptom relief in acute respiratory tract infections, including sore throat. However, while there is moderate scientific support for its use in mild to moderate cases, the evidence is not robust enough to recommend it as a primary or sole treatment for severe tonsillitis, especially cases requiring antibiotics. Safety profiles are generally favorable, but gastrointestinal side effects have been reported. Thus, Andrographis has both traditional and scientific support for alleviating symptoms of tonsillitis, but should be used with caution and not as a substitute for medical care in severe or complicated cases.
