Icariin is a flavonoid compound derived from the plant Epimedium, commonly known as Horny Goat Weed. Its use for male sexual health and vitality has a long history in traditional Chinese medicine. However, in terms of supporting or treating low testosterone specifically, the evidence is primarily preclinical and not robust in humans. Several animal studies suggest that icariin may influence the hypothalamic-pituitary-gonadal axis, potentially boosting testosterone production in rodents. For example, icariin has been shown to increase serum testosterone levels and improve markers of spermatogenesis in some rat studies, possibly by enhancing luteinizing hormone (LH) secretion or upregulating steroidogenic enzymes. There are also indications that icariin can inhibit phosphodiesterase type 5 (PDE5), which may contribute to improved erectile function, indirectly linked to testosterone activity.

Despite these findings, there is currently a lack of high-quality clinical trials in humans directly demonstrating that icariin supplementation significantly raises testosterone levels or effectively treats clinical hypogonadism. Most of the supportive evidence is extrapolated from animal studies or basic cellular research, with limited real-world validation in men. As such, while the scientific rationale exists and preliminary results are promising, the overall evidence quality is low, warranting a rating of 2 out of 5. More rigorous human studies are needed to confirm its efficacy and safety for this indication.
