Eurycoma longifolia, commonly known as Tongkat Ali or Longjack, is a traditional medicinal plant from Southeast Asia that has been historically used as an aphrodisiac and for enhancing male fertility and vitality. In recent decades, several scientific studies have investigated its effects on testosterone levels, particularly in men with low testosterone or related symptoms. Some randomized controlled trials and observational studies have reported that supplementation with Eurycoma longifolia extract can lead to modest increases in serum testosterone levels, improvement in sexual function, and enhanced sperm parameters. The proposed mechanisms include activation of the hypothalamic-pituitary-gonadal axis and inhibition of sex hormone-binding globulin, thereby increasing free testosterone. However, the quality and size of these studies vary, with many being small and of short duration. Systematic reviews and meta-analyses suggest a positive effect, but acknowledge significant limitations due to heterogeneity and potential bias. Despite these limitations, the consistency of positive findings across multiple studies provides moderate scientific support (evidence rating: 3/5) for the use of Eurycoma longifolia in supporting testosterone, particularly in men with suboptimal levels.
