Synopsis

Source of validity: Scientific
Rating (out of 5): 3

HMB (β-Hydroxy β-Methylbutyrate) has a scientific basis for use in supporting healing and recovery after surgery, particularly in the context of reducing muscle loss and improving muscle strength during periods of immobility or catabolic stress. HMB is a metabolite of the amino acid leucine and has been shown to decrease muscle protein breakdown and enhance protein synthesis. Several randomized controlled trials and systematic reviews have investigated the effects of HMB supplementation in surgical patients, older adults, and those experiencing critical illness.

Specifically, studies have assessed HMB (often combined with arginine and glutamine) for its ability to preserve lean body mass and support wound healing after surgery, especially in elderly or malnourished patients. For example, a 2015 meta-analysis (Bear et al., JPEN) concluded that HMB-containing oral nutritional supplements may reduce muscle mass loss and improve recovery outcomes in surgical and critically ill patients. Additionally, some clinical trials report improved wound healing and reduced complications with HMB supplementation, although results are variable and often limited by small sample sizes and heterogeneity in patient populations.

Overall, while evidence is not unequivocal and more large-scale, high-quality studies are needed, there is moderate scientific support for the use of HMB in aiding recovery from surgery, particularly where muscle loss is a concern.
