Alpha-ketoglutarate (AKG) is an intermediate in the Krebs cycle and plays a role in cellular energy metabolism. Its use in supporting recovery after surgery has some scientific basis, though the evidence is limited and not robust. Several clinical trials, primarily in the context of perioperative nutrition, have evaluated AKG (often as part of glutamine- or arginine-enriched formulas) for its potential benefits in wound healing, muscle preservation, and immune function. Some studies suggest that AKG supplementation may reduce muscle protein breakdown and improve nitrogen balance, possibly leading to faster recovery and reduced complications, especially in patients undergoing major surgery or those at risk of catabolic states. The mechanism is thought to involve AKG’s role as a nitrogen scavenger and its contribution to collagen synthesis, which is important for tissue repair. However, most studies have small sample sizes, and AKG is often part of multi-nutrient formulations, making it difficult to isolate its individual effects. Systematic reviews and meta-analyses generally conclude that while there is potential, the evidence is not strong enough to recommend routine AKG supplementation for all surgical patients. Therefore, while there is scientific rationale and some preliminary clinical support, the quality and quantity of evidence remain modest.
