Omega-3 fatty acids, particularly EPA and DHA found in fish oil, have been investigated for their potential role in supporting cardiovascular health, including the prevention and management of strokes. There is a scientific rationale for their use: omega-3s are known to exert anti-inflammatory, antithrombotic, and lipid-lowering effects, which theoretically could reduce the risk of ischemic stroke. Several observational studies have found an association between higher fish or omega-3 intake and reduced stroke risk. However, large randomized controlled trials (RCTs) have produced mixed results. For example, the VITAL trial (2018) and the ASCEND trial (2018) did not show a statistically significant reduction in stroke incidence with omega-3 supplementation in the general population. Some meta-analyses suggest a modest benefit, but the effect size is small and may not be clinically significant for secondary prevention or acute stroke treatment. Importantly, omega-3 supplementation is not a replacement for established stroke prevention strategies such as blood pressure control, anticoagulation (when indicated), or statin therapy. Current guidelines do not recommend omega-3s specifically for stroke prevention or treatment, but they may be considered as part of a heart-healthy diet. In summary, there is some scientific evidence for a preventive role, but it is not strong or definitive, and omega-3s are not standard therapy for stroke.
