Codonopsis pilosula, commonly known as Dang Shen, is a traditional herb widely used in Traditional Chinese Medicine (TCM) to support the Spleen body system, which in TCM is associated with digestion, energy production, and assimilation of nutrients. Historically, Codonopsis has been considered a gentler substitute for ginseng and is often prescribed for conditions like fatigue, poor appetite, and weakened immunity, all of which are linked to Spleen Qi deficiency in TCM theory. Its traditional use is well-documented in classical TCM texts and pharmacopoeias, where it is described as beneficial for enhancing energy (Qi), strengthening the Spleen, and aiding recovery from illness.

While some modern pharmacological studies have explored the potential immunomodulatory and adaptogenic effects of Codonopsis, the majority of evidence for its use in supporting the Spleen system is based on traditional practice rather than rigorous clinical trials or scientific validation. There are preliminary studies showing Codonopsis extracts can stimulate immune responses and may have anti-fatigue effects in animal models, but these do not directly translate to proven benefits for the human digestive system or the Spleen as conceptualized in TCM. Therefore, while Codonopsis is a staple in TCM for Spleen support, the strength of evidence remains primarily traditional with limited scientific backing.
