Andrographis paniculata has a long history of use in traditional medicine systems across Asia, particularly in Ayurveda and Traditional Chinese Medicine, for treating respiratory tract infections, including sore throat. In recent decades, several clinical studies have investigated its efficacy for upper respiratory tract infections (URTIs), which commonly present with sore throat as a symptom. Systematic reviews and meta-analyses—such as those published in the journal Phytomedicine (2017) and the Cochrane Database of Systematic Reviews (2004, updated 2017)—have found moderate evidence that standardized extracts of Andrographis paniculata can reduce the severity and duration of sore throat and other URTI symptoms when compared to placebo. The presumed mechanisms of action include anti-inflammatory and immunomodulatory effects, as well as possible antiviral activity. However, most clinical trials evaluated Andrographis as part of combination herbal products (such as Kan Jang) or as monotherapy in the context of broader respiratory illnesses, not specifically isolated sore throat. While the overall safety profile is good and the evidence for symptomatic relief is promising, the methodological quality and sample size of clinical studies are variable, and further large-scale, well-controlled trials are needed for definitive conclusions.
