Andrographis paniculata, commonly called andrographis, has been traditionally used in Ayurvedic and Traditional Chinese Medicine for respiratory tract infections, including colds and flu. In modern research, several clinical trials and systematic reviews have evaluated andrographis for acute upper respiratory tract infections, showing modest benefits in reducing symptoms such as sore throat, cough, and nasal congestion. However, the evidence specifically for sinus infection (acute sinusitis or rhinosinusitis) is limited. A few studies included participants with symptoms consistent with sinus infection, but did not isolate this group or specifically assess sinusitis as a distinct condition.

Most of the available data aggregate sinusitis with broader upper respiratory conditions. The proposed mechanism involves andrographolide, the primary active compound, which is thought to have anti-inflammatory, antiviral, and immune-modulating properties. The World Health Organization has recognized andrographis for symptomatic relief of uncomplicated upper respiratory tract infections, but not specifically for sinus infections. There is a lack of high-quality, large-scale randomized controlled trials directly investigating andrographis for sinusitis. Thus, while scientific evidence supports andrographis for general upper respiratory symptoms, its specific efficacy for sinus infection is less clear and merits a moderate evidence rating.
