The primary scientific rationale for its use lies in its function as a reversible, competitive inhibitor of acetylcholinesterase, the enzyme responsible for the breakdown of acetylcholine in the brain. By inhibiting this enzyme, galantamine increases the levels of acetylcholine, a neurotransmitter essential for memory and cognitive function, which is deficient in Alzheimer's and related dementias.

Numerous randomized, double-blind, placebo-controlled clinical trials have demonstrated that galantamine provides statistically significant, albeit modest, improvements in cognitive function, activities of daily living, and global clinical state in patients with Alzheimer’s disease (e.g., Tariot et al., 2000; Wilcock et al., 2000).
