There is emerging scientific interest in the role of vitamin D in schizophrenia, but the evidence remains limited and inconclusive. Observational studies have found that individuals with schizophrenia frequently have lower serum vitamin D levels compared to healthy controls. Some research suggests that vitamin D may play a role in neurodevelopment, neurotransmitter synthesis, and immune modulation, all of which could be relevant to the pathophysiology of schizophrenia. A few small randomized controlled trials and meta-analyses have investigated whether vitamin D supplementation improves psychiatric symptoms in people with schizophrenia. These studies have yielded mixed results, with some reporting modest improvements in negative symptoms or cognitive function, while others found no significant benefit. Importantly, no large-scale, well-powered clinical trials have established definitive efficacy for vitamin D supplementation in treating core symptoms of schizophrenia. Current clinical guidelines do not recommend vitamin D as a primary treatment for schizophrenia, but supplementation may be considered in patients who are found to be deficient, given its general health benefits, especially for bone health. Overall, while the biological rationale and some preliminary data provide a basis for further research, there is insufficient high-quality evidence to support routine vitamin D supplementation specifically for the treatment or prevention of schizophrenia at this time.
