Omega-3 fatty acids, particularly eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), have been investigated as adjunctive treatments for schizophrenia based on their role in brain structure and function. Some early studies and meta-analyses suggested mild benefits, especially for patients in the early stages of psychosis or those with low baseline omega-3 levels. Proposed mechanisms include modulation of neurotransmitter systems, anti-inflammatory effects, and support for neuronal membrane fluidity. However, more recent and larger randomized controlled trials (RCTs) have produced mixed or negative results. For example, a 2018 Cochrane review concluded that the evidence for omega-3 supplementation in improving core symptoms of schizophrenia is weak and inconsistent. While some small studies report improvements in negative symptoms or prevention of psychosis onset in high-risk individuals, these findings have not been robustly replicated. Guidelines from major psychiatric associations do not currently endorse omega-3 fatty acids as a primary or standard adjunctive treatment for schizophrenia. In summary, there is some scientific interest and a modest evidence base, but overall, the quality and consistency of evidence supporting omega-3 fatty acids for schizophrenia is low. More high-quality, long-term studies are needed to determine their true utility in this context.
