Omega-3 fatty acids, particularly those found in fish oil (eicosapentaenoic acid [EPA] and docosahexaenoic acid [DHA]), have a substantial body of scientific evidence supporting their role in the management of rheumatoid arthritis (RA). Numerous randomized controlled trials and meta-analyses have shown that omega-3 supplementation can reduce symptoms of RA, such as morning stiffness, number of tender joints, and the need for nonsteroidal anti-inflammatory drugs (NSAIDs). The proposed mechanism involves the anti-inflammatory properties of omega-3s, which modulate the production of pro-inflammatory eicosanoids and cytokines, thus dampening the inflammatory response that characterizes RA. A 2017 Cochrane review concluded that omega-3 fatty acids may result in a small reduction in pain and disease activity in people with RA, although the effects are modest. Other meta-analyses have similarly shown consistent, though moderate, benefits. Omega-3s are not considered a replacement for disease-modifying antirheumatic drugs (DMARDs), but they can be a useful adjunct for symptom control. The use of fish oil for inflammatory conditions also has some traditional roots, particularly in cultures with high fish consumption, but the current application in RA is primarily driven by scientific investigation. Safety profiles for omega-3s are generally favorable, with mild gastrointestinal side effects being most common. Overall, omega-3 fatty acids are scientifically validated as a supportive therapy for RA, with an evidence rating of 4 out of 5.
