Boswellia serrata, also known as Indian frankincense, has a long history of use in traditional Ayurvedic medicine for inflammatory conditions, including arthritis. Scientific studies have started to validate some of these traditional uses, particularly for inflammatory joint diseases such as rheumatoid arthritis (RA). Research indicates that Boswellia extracts contain active compounds called boswellic acids, which are believed to inhibit pro-inflammatory enzymes such as 5-lipoxygenase, thereby reducing the production of leukotrienes involved in inflammation. Several randomized, double-blind, placebo-controlled trials and systematic reviews have been conducted, primarily in osteoarthritis but also in RA. Some small clinical studies in RA patients have shown symptomatic improvements in joint swelling and pain compared to placebo. However, most of the clinical evidence for Boswellia's efficacy is of moderate quality, with relatively small sample sizes and short durations. Meta-analyses suggest it may provide modest benefit in reducing pain and improving function, but larger, well-designed trials are still needed to confirm these effects specifically in RA. Overall, there is moderate scientific support for the use of Boswellia in RA, and it is generally considered safe, but it should not replace standard medical therapies.
