Algal oil is a plant-based source of omega-3 fatty acids, primarily providing docosahexaenoic acid (DHA) and, in some formulations, eicosapentaenoic acid (EPA). The rationale for using algal oil in managing rheumatoid arthritis (RA) stems from the established anti-inflammatory properties of omega-3 fatty acids, which have been shown in several studies to reduce inflammation and potentially alleviate symptoms in autoimmune conditions like RA. Traditionally, marine oils such as fish oil have been more extensively studied and used for this purpose. Algal oil is considered an environmentally sustainable and vegan alternative to fish oil, making it attractive for certain populations. Clinical trials specifically evaluating algal oil in RA are limited. Most existing evidence is extrapolated from studies on fish oil, which have shown modest benefits in reducing joint pain, morning stiffness, and the need for non-steroidal anti-inflammatory drugs (NSAIDs) in RA patients. A few small studies and pilot trials suggest that algal oil can increase blood levels of DHA and EPA similarly to fish oil, but direct evidence for symptom relief in RA remains sparse. Therefore, while the theoretical basis and indirect evidence are present, direct scientific validation for algal oil’s effectiveness in RA is currently weak. It may be considered as an adjunct, especially for those seeking plant-based options, but should not replace conventional therapy.
