Coenzyme Q10 (CoQ10) has been investigated for its role in supporting reproductive health, particularly in relation to fertility. CoQ10 is a naturally occurring antioxidant that plays a crucial role in mitochondrial energy production. In women, several studies suggest that CoQ10 supplementation may improve oocyte (egg) quality, especially in older women or those with diminished ovarian reserve. For example, randomized controlled trials have demonstrated that CoQ10 supplementation can improve ovarian response, embryo quality, and, in some cases, pregnancy rates in women undergoing assisted reproductive technologies such as IVF.

In men, CoQ10 supplementation has been associated with improvements in sperm concentration, motility, and overall sperm quality. A 2013 meta-analysis concluded that CoQ10 supplementation significantly improved semen parameters in men with idiopathic infertility, likely due to its antioxidant properties reducing oxidative stress in sperm cells.

While the data are promising, most studies are small and further research is needed to determine the true clinical impact on live birth rates and long-term outcomes. Nonetheless, the available evidence supports the use of CoQ10 as a potential adjunct in fertility treatment for both men and women, justifying a moderate evidence rating.
