Specialized pro-resolving mediators (SPMs) are a group of bioactive lipid compounds derived from omega-3 and omega-6 fatty acids, including resolvins, protectins, and maresins. SPMs play a crucial role in actively resolving inflammation, as opposed to merely suppressing it. In the context of psoriasis, a chronic immune-mediated inflammatory skin disease, preclinical research has demonstrated that SPMs can reduce inflammatory responses and modulate immune cell activity relevant to psoriasis pathology. For example, animal studies have shown that administration of SPMs can decrease psoriasiform skin inflammation by reducing neutrophil infiltration, cytokine production, and keratinocyte hyperproliferation.

However, clinical evidence in humans remains very limited. There are no large-scale randomized controlled trials evaluating the efficacy of SPM supplementation in patients with psoriasis as of June 2024. Some small pilot studies and case reports suggest potential benefits, and SPMs are increasingly discussed in dermatological research as promising adjuncts to current therapies. Despite this, the evidence base is still at an early stage, primarily preclinical or extrapolated from related inflammatory conditions. Therefore, while there is a scientific rationale and emerging preliminary data, the current level of evidence supporting SPMs for psoriasis is low (rated 2/5). Further well-designed clinical trials are needed to establish safety, optimal dosing, and real-world effectiveness in psoriasis management.
