Vitamin D has been investigated for its potential role in supporting or treating prostatitis, particularly due to its immunomodulatory and anti-inflammatory properties. Several preclinical studies have shown that vitamin D can modulate immune responses and reduce inflammation in prostate tissue. Some observational studies have also noted that men with chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) tend to have lower serum vitamin D levels compared to controls. These findings have led to the hypothesis that vitamin D supplementation might help alleviate prostatitis symptoms or reduce inflammation.

However, the clinical evidence supporting vitamin D supplementation specifically for the treatment of prostatitis remains limited. A few small pilot studies and case series have reported symptomatic improvement in prostatitis patients receiving vitamin D supplementation, but these studies often lack rigorous design, have small sample sizes, or do not include control groups. There are currently no large-scale, randomized controlled trials that definitively demonstrate a benefit of vitamin D in treating prostatitis.

In summary, while there is a plausible scientific rationale and some preliminary evidence suggesting a potential benefit of vitamin D in prostatitis, the overall strength of the scientific evidence is weak to moderate at best. More robust clinical research is needed before vitamin D can be routinely recommended for this indication.
