Vitamin B9, in the form of folinic acid or its more common form folic acid, is strongly supported by scientific evidence for use in pregnancy. Numerous large-scale studies and clinical guidelines endorse the use of folic acid supplementation before conception and during early pregnancy to significantly reduce the risk of neural tube defects (NTDs) such as spina bifida and anencephaly in the developing fetus. Folinic acid is a bioactive form of folate and can be used especially in individuals with certain metabolic or absorption issues.
Health authorities, including the World Health Organization (WHO), the Centers for Disease Control and Prevention (CDC), and the American College of Obstetricians and Gynecologists (ACOG), recommend women of reproductive age take 400 micrograms of folic acid daily, beginning at least one month before conception and continuing through at least the first trimester. Folinic acid, though less commonly used than folic acid, also contributes to these protective effects and is sometimes prescribed for those who have difficulty metabolizing folic acid.
Beyond NTD prevention, adequate folate status in pregnancy is associated with reduced risk of other congenital anomalies, preterm birth, and maternal anemia. The mechanism is well-understood, as folate is essential for DNA synthesis, cell division, and tissue growth, all of which are critical during fetal development. Thus, the use of Vitamin B9 (folinic acid/folic acid) in pregnancy is among the most robustly validated examples of a vitamin supplement in clinical medicine.
