Acetyl L-carnitine (ALCAR) has been researched for its potential role in supporting neuropsychiatric conditions, including Post Traumatic Stress Disorder (PTSD). The basis for its use is scientific rather than traditional. ALCAR is a naturally occurring compound involved in mitochondrial energy metabolism and has demonstrated neuroprotective, antioxidant, and neurotrophic effects in preclinical studies. There is some evidence from animal models suggesting that ALCAR may reduce symptoms related to stress and trauma by modulating glutamatergic neurotransmission and supporting neuronal plasticity. Additionally, ALCAR has shown antidepressant and anxiolytic-like effects in both animal and limited human studies, which may be relevant given the overlapping symptoms between depression, anxiety, and PTSD. However, direct clinical evidence supporting ALCAR specifically for PTSD in humans is sparse. A few small pilot studies and case reports have explored its adjunctive role, often in combination with other treatments, and have suggested potential benefits such as improved mood, cognitive function, and reduced anxiety. These findings are promising but far from conclusive. The majority of existing research focuses on depression, cognitive impairment, and neuropathic pain rather than PTSD directly. In summary, while the rationale for ALCAR use in PTSD is based on its neurobiological effects and some preclinical and preliminary clinical evidence, robust, large-scale randomized controlled trials are lacking. Therefore, the scientific support exists but is weak at present, meriting a low-to-moderate evidence rating.
