Andrographolide, the main bioactive component of Andrographis paniculata, has been investigated for its effects against respiratory infections, including pneumonia. Several preclinical studies demonstrate that andrographolide exhibits anti-inflammatory, immunomodulatory, and antimicrobial properties, which could be beneficial in the context of pneumonia, a condition characterized by lung infection and inflammation.

A number of in vitro and animal studies have shown that andrographolide can inhibit inflammatory cytokines and reduce oxidative stress in lung tissue. Some animal models of bacterial or viral pneumonia have reported reduced lung injury and improved survival rates when treated with andrographolide or extracts of Andrographis paniculata. Limited clinical trials have explored its use in upper respiratory tract infections, with some evidence suggesting a reduction in symptom severity and duration; however, direct evidence in human pneumonia patients is sparse.

A systematic review (Coon & Ernst, 2004; Panossian et al., 2021) notes that while andrographolide and its parent herb have a strong record in traditional medicine for respiratory infections, robust clinical trials specifically for pneumonia are lacking. Thus, while scientific rationale and preclinical data are promising, high-quality human studies are limited, leading to a moderate evidence rating.
