Vitamin B6 (pyridoxal 5'-phosphate) has been studied for its role in the management of premenstrual syndrome (PMS), including PMS Type S, which is characterized by symptoms related to serotonin imbalance such as mood swings, irritability, anxiety, and depression. The scientific rationale for using Vitamin B6 lies in its role as a coenzyme in the biosynthesis of neurotransmitters, particularly serotonin and dopamine. Several randomized controlled trials and meta-analyses have investigated the efficacy of Vitamin B6 in alleviating PMS symptoms. A 1999 meta-analysis published in the British Journal of Obstetrics and Gynaecology found that Vitamin B6, at doses up to 100 mg/day, was more effective than placebo in reducing overall PMS symptoms, with the most benefit seen in mood-related symptoms. Additionally, a 2017 Cochrane review concluded that there is some evidence of benefit, but noted that the quality of the studies is variable and the effect size is modest. Side effects are generally mild at recommended doses, though higher doses can cause neuropathy. While not universally endorsed as a first-line treatment, Vitamin B6 is considered a reasonable option for women with PMS, particularly where mood symptoms predominate.
